Copy of Student ID(Graduates submit a copy of their Certificate of Degree / Diploma)
3.鑑輔會鑑定證明影本
Copy of Disability Identification
4.身心障礙證明正、反面影本(無者免附)
Copy of the front and back of Disability card
5.歷年成績單正本(須含112學年)
Full Academic Transcript(Include Academic Year 2022)
111學年度以後入學(包括111學年度)

申請期限 Application deadline：
即日起至113年10月31日止(逾期恕不受理)，請將上列準備資料備妥，
